Case Series/Study

(CS-109) Application of Fish Skin Grafts on persistent lower extremity ulceration

Introduction: Patient is a 69-year-old female who presented for evaluation of a persistent left anterior leg wound present for over one month of traumatic margin. Patient had previously undergone vein stripping in the past and also reports that she had prediabetes present. Patient had been using topical mupirocin and taken courses of doxycycline and cipro prescribed by urgent care. She now presents because the prior modalities failed to help. Past medical history is pertinent for asthma, borderline diabetes mellitus type II. Past surgical history pertinent first status post bilateral vein stripping.

Methods: Patient underwent full thickness biopsy and debridement of the wound to exclude malignancy as well as to remove any biofilm present. Patient was seen approximately seven days later. At the time results of her hemoglobin A1c were 5.6. Biopsy was negative. Patient underwent wound bed preparation on this visit for a planned skin substitute application. Patient presented the following week at which time she was treated with the Kerecis Omega3 Micro approximately 4cm². The skin substitute was applied in the usual sterile fashion and covered with a bordered silver foam dressing.

Results:

Patient presented the following week at which time the wound was noted to be 30% smaller after the application of one piscine graft. Patient underwent a second application of Kerecis Omega3 Micro 4cm². Initial wound size was 2.0cm x 0.7cm x 0.7cm in depth.

The wound was 30% smaller after the first application. Patient was followed weekly thereafter. And was noted to have full closure of the wound approximately five weeks after the second graft.
